Dear  Parents / Guardians:

We ask that you please fill out the attached dismissal form for your Pre-K thru 6th grade student(s).   Please have this information to us no later than September 2nd.  You can drop off, mail,  fax to 698-2345,  scan this form or email your child(rens) daily dismissal plan to  The form is also available on our website and is included in our summer mailing. 

It is only necessary to provide this information once. 

Thank you for your prompt attention to this.   It is very important to have this information before the first day of school so that our dismissal will follow the safety guidelines that we are putting in place.
